Your privacy is respected. Any personal information collected will be used only in accordance with current data protection legislation applicable to England. The NHS Leadership Academy (the “Academy”) which is part of the National Health Service Trust Development Authority (“NHS TDA”, also referred to jointly with Monitor as “NHS Improvement”) is registered with the Information Commissioner’s Office (www.ico.gov.uk), registration number is Z2950066. Collection and Use of Personal Information
You will be asked to provide some personal information such as your name, email address, job role, organisation and where you live. We collect and store this information in order to perform statistical analysis and to conduct research studies. This information is kept strictly confidential.
The email address you provide may be used to automatically generate reminders based on the preferences you set and control within your profile. You can change these settings at any time.
You may be invited, by email, to take part in additional research or evaluations about the Healthcare Leadership Model and/or the leadership behaviour observation app. Participation in this research / evaluation is entirely voluntary.
Access to Information We Collect
Your contact information is accessible to the NHS TDA (though the Academy) and the operators of the app. The operators include a limited number of specifically authorised employees. All operators are employed under contracts that set down strict confidentiality regulations governing their behaviour. Non attributable response data may be accessed by authorised personnel involved in the management of the project, or for research purposes.
Processing and Storage of Information We Collect
The NHS Leadership Academy is registered with the Information Commissioner’s Office (Registration number is Z2950066).
Using the app is entirely voluntary. There are no negative consequences if you decide not to use the app.
How We Keep Our System Secure
Our system has security measures in place to protect against the loss, misuse and alteration of the information under our control. These security measures include firewalls, backing up of data and, where appropriate, encryption and secure internet transactions.
Non-Identifiable Information
This app automatically collects certain non-identifiable information, such as the Internet Protocol (IP) address of your computer/device, the IP address of your Internet Service Provider, the date and time you access the app, the Internet address of the Web site from which you linked directly to the app, the operating system you are using, the sections of the app you visit, the app pages accessed and information viewed, and the materials you post to or download from the web site. This non-identifiable information is used for reporting and analysis purposes and to improve the functionality of our app. Your non-identifiable information may be disclosed to others and permanently archived for future use.

The names, images and logos identifying the NHS TDA (though the Academy) and the Healthcare Leadership Model are proprietary marks. If you wish to copy or use our logo or graphics, you must get prior approval from the NHS TDA’s (through the Academy) branding team. If permission is granted, you will also be provided with guidance on using our branding. These guidelines must be adhered to.
